You will need a install CD and an extension disk. I have used the Blade XFS install CD from http://people.debian.org/~blade/XFS-Install/ and his RAID and LVM extension disks from http://people.debian.org/~blade/install/lvm/xfs-fork
I have modified the Root on LVM on RAID documentation from http://www.midhgard.it/docs/index_en.html. The things that I have added is highlighted in yellow.

To create a server of different types I have used the following structure:
For servers that are extremely critical var and swap might be on its own partitions without LVM. On the other hand I have not had any problems with LVM so far.
- /dev/md0 128 MB ext2 mounted as /boot
- /dev/md1 1024 MB xfs mounted as /
- /dev/md2 rest MB LVM
- /dev/vg00/var 256 MB xfs mounted as /var
- /dev/vg00/swap 2*RAM MB used as swap
